Coming out of hospital after an operation is always a worrying time both for the person who has been in hospital and for their family members. Most people are offered a Reablement Package on discharge from hospital usually for a few weeks of live-out support to help recovery and some daily tasks.
However, an NHS Reablement Package does not offer the reassurance and security of someone living in the home overnight. Home from Hospital offers the reassurance of having a fully- vetted, kind and caring Sharer who moves in for a temporary, agreed period whilst the person recovers. The Sharer will sleep in the home overnight and help with cooking, shopping, light household tasks and offer some company too.
A Home from Hospital arrangement sits nicely alongside the Reablement Package and will benefit people who are recuperating from an illness or operation and speed up their recovery.
Home From Hospital supports people leaving hospital in the London Boroughs by introducing a live-in Sharer to a Householder who will move in for a short period of time whilst the Householder is recuperating from an operation or illness.
The Householders are normally people who live alone and are returning home from hospital after an operation, such as a hip operation, a fracture, cancer treatment or a chronic illness, and would benefit from the reassurance of someone sleeping in the house and some practical support during their period of recovery.
The service is very flexible and will also suit couples who need some support whilst one is recovering from a stay in hospital.
Sharers offer practical help and company and are NOT permitted to do any personal care, including dressing or administering medication. A care package will normally sit alongside this service.
Home From Hospital offers Householders the reassurance of someone sleeping in their home at night plus up to 15 hours of low-level practical support and company during the week.
The kind of help offered is cooking meals (up to 5 evenings each week), shopping, light housework, taking bins out, watering plants and running errands plus the natural company from sharing a house and some meals together.
The hours of support will be pre-arranged before the Homeshare commences. In exchange for this support, the Sharer will be given a bedroom in the home to use as their personal space for the duration of the arrangement.
Sharers are not paid employees and usually offer their support in the evenings and at weekends. They are generally out of the property during the working day.
They will normally sleep in the home every night each week (except by arrangement) but are allowed to go out on two evenings per week and go away for up to one weekend each month with prior arrangement.
Sharers are responsible, kind, friendly and caring people from all walks of life who are looking for a temporary place to stay and offer practical help in return.
Sharers are fully vetted; they will have been interviewed, completed an Enhanced DBS (criminal records) check, as well as Credit and Right to Rent checks and will have supplied at least two references.
Sharers cannot take holiday during the period they are helping, but can have one weekend away per month by prior arrangement with the Householder.
The Sharer does not contribute towards utility bills but are responsible for the cost of their own food.
Sharers are NOT permitted to do any personal care…
To qualify for this arrangement, the Householder must have a domiciliary care package in place to sit alongside the Homeshare or they must be able to manage their personal care independently.
Personal care includes activities such as getting up, going to bed, going up and down stairs, getting dressed, washing, showering or bathing, and emptying commodes.
There is a simple Registration Process that the Householder will be required to follow:
- Complete the Home from Hospital Registration Form for the team to assess for suitability.
- Email photos of the property to include the bedroom the Sharer will be using, as well as the bathroom and kitchen.
- We will also require a photo of the Householder for their Householder profile
- Householder proof of address.
We will require enough notice to introduce a Sharer to you so for planned operations please let us know once you have a date for your operation; for emergency Sharers with not much notice, please call us to discuss before completing the Registration Form.
